Reporting and acquittals

Each project funded by Healthway is required to complete an acquittal report at the conclusion of their funding. The comprehensiveness of the evaluation requirements are determined by the type of project and level of funding provided.

Healthy Partnership Program

At the conclusion of your project, you will be asked to complete an acquittal form outlining how you achieved your objectives and key results from the project (annual progress reports are also required for multi-year projects). You will also need to provide a signed financial statement detailing how the funds were spent.

Healthy Spaces

At the conclusion of your funding, you will be asked to complete an acquittal form which can be found below:

Healthy Communities

At the conclusion of your project, you will be asked to complete an acquittal form including an evaluation report (only for projects above $5,000) outlining how you achieved your objectives, and key results from the project (annual progress reports are also required for multi-year projects). You will also need to provide a signed financial statement detailing how the funds were spent.

Healthy Research

At the conclusion of each year of your research project, you will be asked to complete an evaluation report outlining the progress of the research, key findings and other results such as publications arising from the research and knowledge translation activities. Annual financial reports are also required. A reporting template can be found below:
