Personal information collection and use

Healthway may collect personal information as part of its usual business processes. This mainly includes information provided through grant funding application forms, subscribing to eNewsletters, and data available from website access.

Personal information may be provided by organisations submitting a funding application form and consists of personal contact information (as nominated by the organisation) that would allow us to personally contact you, such as your name, home or mailing address, phone number and/or email address.

Personal information that is collected is kept strictly confidential. Healthway only uses the personal information collected to carry out inquiries, dispute resolution, and undertake its core business functions. However, funding applicants are advised that in some cases personal information may be passed to a third party that may be undertaking evaluation or other work on behalf of Healthway or as otherwise specified.

If you subscribe to any services on the Healthway website or provide any comments or feedback via email, then your email address and other personal information you may provide will be maintained on our mailing lists.

Your email address is confidential. When you email us:

• We will record your email address
• We will only use your email address for the purpose for which you provided it
• We will not use your email address for any other purpose
• We will not disclose it to another party without your consent (though from time to time we may disclose your personal information to a party acting on behalf of Healthway)
• Email correspondence sent to this site will be treated as a public record and will be retained as required by the Western Australian State Records Act 2000 and other relevant regulations.

We use website analytical tools to help track links you visit before, during, and after our site as well as overall activity on our website. This information is used to change or improve our website, and your experience. This website contains links to other sites. We are not responsible for the privacy practices of these sites and encourage you to read the privacy statements of each and every website that collects personally identifiable information.

Data quality and updating your details

Healthway maintains and updates the information we hold as necessary or when we are advised by individuals that their personal information has changed. Personal information can be updated by contacting us.

Data security

Healthway takes reasonable steps to protect the information we hold against loss, unauthorised access, modification or disclosure, and against other misuses. The information collected is held on electronic databases and a number of measures are in place to safeguard personal information. Some information is also held on paper files. When no longer required, the information in paper files is destroyed in a secure manner, in accordance with Healthway’s approved procedures. Information stored in our electronic database, when no longer required, is deleted in a secure manner. The database maintains audit trails whenever information is accessed, including amendments or deletions on the database.

How we handle requests for access to personal information

Contact us if you would like to request access to details of the personal information that we hold about you. This will be provided to you free of charge. 

Website privacy information

This site uses Piwik Analytics, an open-source web analytics platform to collect information such as:

• Pages visited
• Browser capabilities (e.g Java, Flash, etc…)
• IP Address
• The previous site visited
• The type of browser and operating system.

Do Not Track request

We honor Do Not Track requests and you can easily opt-out with most browsers: 

• Firefox – Tools  > Options > Privacy > Request that sites not track you
• Chrome – Settings > Show advanced settings > Send a “Do Not Track” request with your browsing traffic
• Internet Explorer – Settings > Privacy > Tracking Protection > Add Tracking Protection
• Safari – Settings > Privacy > Ask websites not to track me.

This site uses the MaxMind GeoLite City database for geolocation. There is a local copy of the Geo-IP database which includes Country, Region (state), and City information. No data is sent to MaxMind or any third party.

Server logs

The server logs all access including the IP Address and Time Stamp of each request. This could lead to personally identifiable information. However, no attempt will be made to identify users or their browsing activities except, in the unlikely event of an investigation, where a law enforcement agency may exercise a warrant to inspect Healthway’s logs.
